Admissions Procedures
Applicants must adhere to the following procedures in order to be considered for admission.
- Complete and submit the Application for Admission.
- Applicants must receive a School Catalog prior to enrollment and sign an acknowledgement of same.
- Applicants must sign the Attestation of High School Graduation/GED form to demonstrate he/she has graduated from high school or has successfully passed a GED or equivalent.
- Applicants for the Massage Therapy Program must meet with an Admissions Staff member; a Heisei Administrator makes all final decisions on acceptance or denial of admission of applicants to the program.
- Applicants are notified of acceptance or denial to the school’s programs.
- Upon acceptance, the applicant must complete the enrollment process
as follows:
- Read and sign the Enrollment Agreement. Note: The student must have read and must understand the contents of the Catalog.
- Applicant must sign the Self-Certification form if making payments to the school for tuition and fees.
- Pay the fees as required as stated in the Enrollment Agreement.
- Submit TB and MMR Clearances.
- Submit a copy of U.S. Driver's License, U.S. Photo I.D. or Alien I.D. Card.
Applicants must adhere to the above procedure in order to be considered for admission. The applicant must have all financial arrangements completed and agreed to as stated on the Enrollment Agreement.
The following items must be submitted and satisfactorily completed prior to commencement:
- Applicant must complete an Application for Admission.
- Applicant must sign Enrollment Agreement.
- Self-Certification form (if applicable).
- Applicant must pay the fees as agreed to on the front page of the Enrollment Agreement.
- Applicant must provide proof of negative tuberculin (TB) test or chest x-ray report and Measles, Mumps and Rubella (MMR) immunization clearance before beginning the program. Refer to the Admissions Requirements for more information about this.
- Applicant must submit A COPY OF HIS/HER VALID U.S. DRIVER'S LICENSE, U.S. PHOTO I.D. OR ALIEN I.D. CARD.
- Applicant must sign an Attestation of High School Graduation; all high school graduates are encouraged to provide a copy of his/her high school transcript. Applicants under the age of 18 MUST PROVIDE A COPY OF HIS/HER HIGH SCHOOL TRANSCRIPT OR GED.
